Your Health. Your Wellness. Our Priority.

Community Health Centers were started as a movement to give back resources and empower communities to take care of themselves. NTACHC is focused on continuing this heritage of community care. It is uniquely positioned to do so by providing high-quality medical services, while serving as a safety net for the lower-income population, to help end health disparities in the community.

In January 2002, a collaboration of area residents, faith-based groups, and health and social services providers founded the Fort Worth Northside Community Health Center, Inc. (FWNCHC) as a 501(c)(3) nonprofit organization to address the health disparities of Fort Worth’s Northside residents. The chronic disease burden was disturbingly high in certain regions within the service area, in a large part as a result of the socio-economic challenges faced by the community. The clinic was then designated a Federally Qualified Health Center (FQHC) eligible for a federal grant.

In 2005, FWNCHC assumed control of the health center in conjunction with the University of North Texas Health Science Center and John Peter Smith Health Network. In 2010, the Board renamed the corporation as North Texas Area Community Health Centers, Inc., – a name fitted more with the new service area and strategic direction.

Two additional locations, Southeast Fort Worth and Arlington, were opened in areas designated as medically underserved population areas.

NTACHC’s capacity to continue meeting the growing numbers especially at our Northside Community Health Center was limited until recently. In September 2018, NTACHC opened its new 33,800 square foot Northside Community Health Center. The Northside Center offers comprehensive primary care services, behavioral health care, diabetes management, lab services, and a pharmacy.

We offer a coordinated team-based approach to healthcare, in which a team of medical professionals work together to provide for your primary health care needs (acute, chronic, and preventative) and assist in arranging appropriate care with other qualified professionals and/or specialists to ensure your continuity of care. Our teams at NTACHC include physicians, physician assistants, nurses, medical assistant, pharmacists, social workers, and educators. This model results in more personalized, coordinated, effective and efficient care.

NTACHC works closely with you and your family to understand and respect your unique needs, culture, values, and preferences. We actively support you in learning to manage and organize your own care at the level you so choose. Recognizing that patients and families are core members of the care team, NTACHC ensures that your family is a fully informed partner in establishing care plans.

NTACHC delivers accessible services to you and is responsive to your preferences regarding access. We demonstrate a commitment to quality and quality improvement by ongoing activities such as using evidence-based medicine and clinical tools to guide shared decision making with you and your family.
